• Please allot about 1 hour for your appointment.
• For pregnancy tests, plan to provide a urine sample.
• Your appointment is not confirmed or scheduled until we call you.
  

Due to COVID-19, we are currently requiring the following:

• All patients must be screened before receiving services.
• All patients must wear a face mask during their appointments.
• We are following the recommended guidelines of 6 ft. social distancing.
• We can only provide services to one patient at a time.
• No children or friends are permitted to accompany patients.
